Members pick their own caregiver and decide how care fits around their life at home. Family can help, but the member has the final say. That makes Roost a natural fit for programs like Veteran-Directed Care, where veterans hire their own caregivers. If a member’s caregiver is away, the visit goes first to someone they already know, and over time that caregiver learns what normal looks like for them.

Roost is building community health infrastructure: trained caregivers and nurses close to where members live, all on the Roost platform. It means more members can be looked after safely at home, with less reliance on hospitals and acute care.

After every visit, the caregiver completes a short report on the key areas of their day, from medication and eating to mobility and mood, each compared with the member’s own normal. Horizon AI flags changes early, and when a serious change is reported, a Roost coordinator calls the family within the hour and prompts a call to the member’s doctor when needed. Every decision about care stays with a person.

ready for billing.
Caregivers clock in and out at the member’s home, and every visit is recorded to the electronic visit verification standard. Claims are raised from that verified record under each plan’s rules. Screening, training and certificates are tracked by state and role, and records can be exported for audits at any time. Care records are protected under HIPAA, and every access is logged.
